Assist Personal Activities – High Intensity Support

There are two levels of support for people requiring help with everyday activities: standard and high intensity. Assist Personal Activities – High Intensity Support offers specialised and professional support to people with disabilities and complex care requirements. Participants receive help from professional support workers with specialised qualifications and experience that will cater to their specific needs.


A higher intensity support might be paid where a participant needs help from a specialist with extra capabilities. An example of this level of support is when a participant requires intensive behaviour support to complete personal activities. 


For example, when:



  • Frequent (at least 1 instance per shift) assistance is required to manage challenging behaviours that require intensive positive behaviour support
  • Frequent (at the least 1 example consistent with shift) assistance is required to manage tough behaviours that require in-depth positive behaviour help
  • Continual active support is required due to high medical support needs (such as unstable seizure activity or respiratory support)
  • Continual active support is needed because of excessive medical help needs (such as unstable seizure activity or breathing support).


Assist Travel/Transport

We provide travel and transport services to people who need help getting around. Assist Travel/Transport is designed for participants who cannot use public transport due to their disability or immobility. Our vehicles are modern, clean and comfortable, to get our participants from A to B in comfort.


Our transport assistance professionals are highly qualified and compliant with valid driver’s licence and police background checks. They are available any time of day to take you to and from the shops, work, study, social engagements, or your favourite coffee shop. We aim to make every day mobility much easier and safer for our participants.


Assistance with Daily Personal Activities

We can help keep you or your loved one healthy and mobile with our Assistance with Daily Personal Activities service. We recognise that the assistance required varies from one individual to another; therefore, we will pay close attention to your NDIS plan and work with you and your family to make sure our assistance helps you achieve your personal goals and needs.


Our team of qualified and experienced professionals provides help with many daily activities, including:



  • House chores
  • Shopping & errands
  • Attending appointments.


Our support staff will go above and beyond to assist you while you are at home, at your job, studying or socialising.

Assistance with Social and Community Participation

We offer Assistance with Social and Community Participation supports to help you improve you or your loved one’s ability to participate in community, social and recreational activities. Receive one-on-one, attentive care when out in the community with our trained disability support workers.


We have earned a solid reputation for providing individuals with highly curated social and community participation. Let us support you in your favourite activity, from visiting the park to shopping, bowling, painting classes, dance classes, yoga class, or to wherever your interests lie.

Community Access

Recreation and leisure activities are an important part of maintaining a good quality of life. For individuals with a disability, it provides a pathway for social opportunities, which helps with skill development and competencies.


We provide on-hand assistance with attending family events, as well as leisure activities. It’s important to us that when you are attending recreational activities, you are receiving both physical and emotional support to stay connected and involved.


How we provide Community Access services:

  3. Social support in the community
  4. Social skill development

Community Nursing Care Services

Community Nursing Care Services are available to patients and NDIS participants of all ages whose care is more appropriately delivered in the home setting. They might:


  • have complex health needs
  • have acute or chronic conditions
  • require palliative care.


We aim to provide professional nursing care, advice, and support to housebound patients by agreeing on an individualised plan of care with the patient. We will help you to improve your quality of life, maximise independence and, wherever possible, prevent inappropriate admission to and facilitate safe and timely discharge from the hospital. Our comprehensive services are designed to optimise your health, empowering you to remain at home with the maximum level of independence.

Diabetes Management

Diabetes is a chronic, serious and complex condition that affects the entire body. It can severely impact a person’s morale and standard of living. Although there is no cure for diabetes, diabetics can benefit from personalised help to learn about their condition and how to effectively manage it.

Diabetes Management
requires daily self-care, or assistance to do so. Failure to do this care can result in complications that significantly impact wellness, quality of life, and life expectancy. Our expert clinical care provides the support you need to improve blood glucose control and reduce diabetes-related complications. Our goal is to help avoid complications and maintain healthy lifestyle behaviour such as:

  • maintaining an appropriate weight
  • eating a balanced diet; and
  • increasing physical activity, as you’re able.


We offer a comprehensive home-based diabetes support and management program designed to help patients with diabetes who are:


  • worried about starting insulin
  • unsure what to do with their blood sugar when it is high or low
  • don’t know how to manage their lifestyle to improve diabetes
  • control
  • need help mapping out a diet plan customised to their needs
  • don’t know how to check their feet to prevent diabetic foot complications
  • already suffering from diabetic foot ulcers
  • want to reduce the stress of frequent trips to the hospital to treat diabetes-related complications.

  1. Development of a patient/participant assessment and comprehensive care plan
  2. Screening and monitoring blood glucose levels, weight, and blood pressure
  3. Insulin administration and management
  4. Diabetes education customised to patient/participant needs

  6. Active lifestyle education and counselling
  7. Monitoring for diabetes-related complications
  8. Diabetic foot care, counselling, and support




Continence Assessments And Management

This service manages continence assessments and provides recommendations for ongoing management. In conjunction with your Coordinator of Supports, we will source suitable containment products and provide them to you, for your comfort.


Continence Assessments and Management also offers education and training to health and care professionals, as well as caregivers and family members, of the participant/client.


Many of our participants in this service live far away from major services. For this reason, we travel into the communities and take this service to our people.

Daily Tasks/Shared Living

We provide assistance with, or supervision of, daily tasks in a shared living environment. We assist these participants to perform their daily tasks as independently and efficiently as possible. By helping with day-to-day tasks, our caregivers allow clients to maintain their regular daily routine. Surrounded by their belongings and a familiar atmosphere, our clients receive the encouragement they need to enjoy their daily activities and to continue living well at home.

In-Home Supports

In-Home Supports is an alternative to out-of-home care, such as skilled nursing facilities, assisted living, or board and care facilities. It helps to pay for services to eligible aged, blind and disabled individuals who are unable to remain safely in their own homes.


What kind of help is available?


In-Home Supports covers the most common activities of daily living needed by those with disabilities. The different activities are assessed according to the needs of the individual. They may include, but are not limited to:



  • Housekeeping
  • Grocery shopping
  • Meal preparation and clean up
  • Transportation to medical appointments
  • Spoon-feeding
  • Help with breathing equipment
  • Toileting or incontinence-related needs
  • Bathing and hygiene tasks
  • Dressing assistance
  • Medication management
  • Tooth, nail, and hair care.


Innovative Community Participation

Our Innovative Community Participation service is designed to connect you with enriching programs in your local community. These programs are designed to help you make new friends, develop new hobbies, have fun, all while developing independence and social skills. We believe that every individual knows what works best for them; so, when you make a choice, we work with you!


Our experienced staff adopt a capacity-building, community-based approach by working with participants in their local community. They will help increase the likelihood of uptake and successful transition away from the service to informal supports and/or employment.


Community Participation can be provided to help participants achieve their individual community and social participation goals, such as:

  • going shopping
  • travelling
  • attending a sporting event
  • going for doctors/and or medical appointments.


We can provide one-on-one support to assist people to access and participate in the community throughout the week. Participating in community-based activities improves well-being as you are able to feel connected to people and the greater community. Your well-being is our highest priority. Improved well-being affects different aspects of life including education, employment, health, and satisfaction.

Medication Management

As people age, they can develop multiple chronic conditions, which may require several prescription drugs from more than one prescriber. With this situation, there comes an increased risk of drug interactions, side effects, and accidentally missing or doubling doses. Medication management is an important set of practices to prevent these things from happening.


With our in-home
Medication Management services, you may not need to make the transition from the comforts of home to an assisted-living community, which can be hard on a family. 


Depending on your needs, our qualified staff can help with medication management by:

  • Coming to your home to help administer medications
  • Provide support to caregivers, helping them set medication reminders.

Respite (STA)

We provide emergency respite NDIS supports for families and their caregivers in our respite house. Dedicated professionals provide high-quality care, and free nursing care services are also made available while receiving respite supports. These include, and are not limited to:

  • Injections – eg insulins
  • wound care management
  • support to attend medical appointments with a professional nurse
  • support to attend dialysis
  • after dialysis care.


Respite (STA) is a practical solution for clients who require temporary supports out of home, while allowing your carer to have short-term breaks from their caring responsibilities. This support can be accessed at flexible intervals for up to 14 days at a time, dependent on your needs and requirements.


Respite needs and costs

The cost of respite differs greatly between individuals as it depends on:

  • their needs
  • the length of their stay
  • the level of support needed
  • how inclusive they would like the service.


At a basic level, in-home respite is generally the more affordable option. In-home respite only requires support services and not accommodation charges. However, out-of-home respite is slightly more expensive as it includes the cost of the support services and a portion of your accommodation cost. Sometimes, people opt for out-of-home respite as a reason to go and try new things as a means to reach their goals.

The amount of respite care can vary depending on the individual, their needs, and their situation. Sometimes your primary caregiver may only need a few hours, a day or week. At other times, this could extend to overnight or a few weeks.



Short-Term And Medium-Term Accommodation

Short and medium-term accommodation (STA/MTA) is housing that enables people with very high needs to receive the support they need. STA/MTA will provide funding towards the cost of the physical environment for eligible NDIS participants to live and receive their daily supports.
